We are currently accepting new patients. Please bring along your ID and ask for a GMS1 form.  We will be happy to register you.

To protect our other patients and staff please do not visit our surgery if you are suffering from flu like symptoms, diarrhoea or vomiting in the last four days.  The doctors will be happy to do a telephone consultation.  Ask reception for details.

As from the 1st June, 2016 will will no longer operate an open access surgery.  We have listened to your concerns and changed the way you access the doctors in the surgery.  PLEASE DO NOT QUEUE OUTSIDE THE DOOR FOR AN APPOINTMENT.  Please telephone the surgery or book on line for an on the day appointment.  Thank you.

Help the NHS reduce spending by only ordering what you need on your prescription.  Please let the staff know if you no longer require a medication.  This will enable the NHS to reallocate funding. Thank you for your help.

New Patient Registration

consultation1The doctors welcome new patients who live within our practice area.

If you are registered as a new patient, you will be asked to complete your medical card (or other form supplied by us, if you do not have your medical card) to enable us to obtain your full medical records from your previous GP.

You will also be asked to complete a lifestyle questionnaire and have a health check with our practice nurse before having a consultation with your doctor.

You have the right to express a preference of doctor (e.g. you might prefer a male or female GP) when you join our list and, whilst the practice will endeavour to comply, this may not always be possible. If this were the case, an explanation would be offered.

You are NOT a registered patient with the surgery until you have been offered a health check with our Health Care Assistant.  This appointment will be given when you submit your registration form to our registration clerk with your accompanying documentation Monday to Friday.  A copy of the practice policy with regard to registration required documents may be obtained from reception.

Please contact reception for further information.

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
